I’ve seen teams use a mix of pitch counts (like 100‑120 max per start) and strict innings limits per season, especially for young arms. My cousin’s in the minors, and his team won’t let him throw more than 160 innings in a year—no matter how well he’s pitching. It’s brutal sometimes, but they’ve kept him healthy so far.
