To cut to the chase: under resting conditions, mainstream smartwatches (Apple Watch, Garmin Fenix series, etc.) usually deliver heart-rate readings within ±5 bpm of medical-grade chest straps like the Polar H10. Once you hit intense exercise or the watch isn’t sitting snugly, the error balloons—sometimes by tens of bpm. That’s because optical PPG sensors are thrown off by sweat, arm jitter, and skin tone, whereas ECG patches pick up electrical signals directly and shrug off interference far better. Compare them to hospital-grade blood-pressure cuffs or ECG machines and the watch wins on real-time data—updates every second and instant upload to your phone—but on precision they’re still “trend monitors.” If you need numbers for an official diagnosis, pair the watch with a chest strap or head straight to the hospital for a proper ECG. For everyday health tracking and workout feedback, though, a high-quality optical sensor (Apple Watch Series 8, for example) with proper fit is more than enough.

Yeah man, I recently bought an Apple Watch and a Garmin series too and tried them out. I can tell you the data isn't 100% the same as clinical measurements. Especially during running, the heart rate sensor can slip, get messed up by sweat and movement artifacts, so you get some wonky readings now and then. For sleep tracking, the sensor's vibration detection and slight shifts in the arm overnight sometimes misclassify sleep stages. Still, they're pretty useful for long-term trends; like when you accumulate weekly average HRV or sleep duration data, you can kinda monitor your health with "partial accuracy." I think the best thing is to keep the device in the right position, secure it with a tight strap, and periodically compare the measurement data with a medical device a doctor would use. That way, you can spot sensor errors and see the smartwatch more as an "assistant" for daily motivation and general health trends. Honestly, I've been using a fitness coach who refers to this data too, but they keep it at an "observational" level rather than making a definitive diagnosis.
